Your Adult Social Care Assessment 

Your assessment helps us understand your care needs. 

Please answer honestly so we can get a clear picture of your situation and how we can help. 

There are many support services in Sefton you might not know about. You can find these in the Sefton Charity and Voluntary Sector Directory.

How to Get an Assessment 

If you think you need an assessment, call 0345 140 0845 (Monday to Thursday - 9am to 5.30pm, Friday - 9am to 5pm, closed weekends and Bank Holidays). 

We’ll take your details and someone from our team will contact you to arrange the next steps. 

What Happens During an Assessment? 

A team member will visit you to discuss your daily routine, needs, and interests. You can have someone with you if you like. 

We might ask about: 

  • You, your family, and friends: We’ll learn about you, your hobbies, and your support network. 
  • Your community: We’ll ask about your community activities and any you’d like to join. 
  • Personal care and daily tasks: We’ll discuss your routine and any tasks you need help with. 
  • Managing your healthcare: Tell us about any health conditions, mobility issues, and medications. 
  • Your home: Share your living situation and how it works for you. 
  • Keeping safe: Raise any safety concerns and how we can help you stay safe. 

This helps us decide if you qualify for social care support. 

What Support Can You Get? 

We can help with: 

  • Staying in touch with family and friends 
  • Enjoying your hobbies 
  • Joining community activities 
  • Work, volunteering, or education 
  • Eating well 
  • Personal care, like bathing 
  • Emotional wellbeing 
  • Moving around your home 
  • Keeping your home clean and tidy 

After the Assessment 

We’ll check if your needs meet the criteria for support. You qualify if: 

  • You have care and support needs due to a physical or mental condition. 
  • You can’t achieve two or more outcomes from your needs assessment. 
  • This significantly impacts your wellbeing. 

If you qualify, we’ll create a support plan with you. If not, we’ll connect you to other community services to help you live independently. 

What if I disagree with my Assessor? 

If you think the decision is wrong, ask your assessor to explain. They will review your needs and the criteria. You can request a reconsideration or another opinion. 

Paying for Care 

You may potentially have to pay for care services. For more information, visit our Paying For Care page.

Changing Needs 

We regularly review your needs to ensure you get the right support. If you don't have a review visit schedule and your needs change, you can request a care review.
