Preparing For Adulthood

The transition from childhood to adulthood is often a difficult time of change for many young people and especially those with a disability or special educational need. There are many decisions for a young person and their families to take regarding the future.

This can be a complex stage as support from agencies such as health, education and social care can change from the age of 16-18 and it is important to start preparing for adulthood as early as you can. 

Preparing for Adulthood Guide

We have produced a Preparing for Adulthood Guide which covers education, health, accommodation, independence, community and more to ensure that we can make this exciting but tricky time as smooth as possible.

Transition Coordinators

Sefton Council has two transitions co-ordinators who work with young people from the
age of 14-25 years old.  They provide a person-centred approach when working with young people to create a transition that is clear to all involved and represents the views, opinions, and choices of the individual. To contact the transitions coordinators,
