Your Personal Advisor and Pathway Plan

Your Personal Adviser or ‘PA’ will be your main point of contact with the care experienced service and provide support to help you make a successful transition to adulthood.

When you are 16 you will be allocated a PA from the Care Experienced Team. All young people should know who their PA is and how to contact them if you don't, please contact the Care Experienced Team.

You have the right to PA support up to the age of 25 and the PA will work alongside your Social Worker. It’s your PAs job to make sure you have a Pathway Plan and it is kept up-to-date until you reach 21.Until you reach 21 your PA will see you at least once every two months and keep in touch between visits.

You’ll work with your Personal Adviser to write your Pathway Plan. The plan will include information to help you transition from care to living independently.

We will start developing the Pathway Plan while you are still cared for and it sets out how we will help you to achieve the things you want to do.

You will be fully involved in preparing and reviewing your Pathway Plan and we must review your Pathway Plan with you at least every six months until you reach 21.

In your Pathway Plan we will include information on:

  • education
  • training
  • employment
  • well-being
  • your home
  • family and social relationships
  • money
  • health
  • your ‘self-care’ skills
  • identity

Make sure you talk to your PA about any needs or plans you have so they’re included in your Pathway Plan.
