Our Vision

Sefton Council Adult Social Care Vision

We want people who live in Sefton to live a quality life , as independently as possible for as long as possible. If and when they need it, we want people to have access to good quality support that has a positive impact on their lives.

Adult Social Care Ambition

We want to offer care and support that empowers people to live an independent life, exercise choice and be fully informed.

What we want to see :

  • People are supported to remain independent and to have choice and control
  • High quality support and service for those with complex and neurodiverse needs
  • An early intervention and prevention approach providing the right level of support at the right time
  • Equality for all people in terms of experience and outcomes
  • Carers are well supported and able to achieve their personal outcomes
  • The most vulnerable people living in safety free from harm, discrimination and abuse
  • Working with our health and social care partners to deliver a coordinated response
  • Respect and value our workforce ensuring stability, capacity and sustainability
  • Develop systems for self-assessment so people tell their story only once
  • A Social Care market that understands and responds to the diverse needs of local people, offering flexibility , choice and continuity

Sefton Council Adult Social Care- Our Vision

We will work towards these goals by reducing the reliance on resident care homes and redirect more resources into developing high quality services that support people to remain independent for as long as possible.

This is the aim of our Better at home project.

Better at home  

The goal of Adult Social Care is to help people stay at home with their family and friends. for as long as is possible.

If required we will provide support including , formal care services, advice, information, and access to therapy or community health services. The Better at Home program aims to make this happen. 

Data shows that fewer people should be in care homes in Sefton and more should be supported in the community in their own homes. People move into care homes when they can't be supported at home, haven't recovered from illness after leaving the hospital, or when a short stay in a care home becomes permanent. 

We want to offer more services to give people more choice and control to avoid moving into care homes. This is called "step up" to avoid hospital admission and care home admission in the long term. 

We also need enough good quality home care and reablement services to help people stay at home instead of moving into care homes. 

The better at home program will help achieve this vision on the next 3 years.

The focus will be on supporting people to stay independent for as long as possible and reducing reliance on care homes. This includes developing high-quality services and working with health partners to support hospital discharges. Potential areas to develop include: 

  • Extra care housing 
  • Private extra care housing 
  • Sheltered housing models 
  • Expanding reablement and home care 
  • Early intervention strategy with increased support for informal Carers, advice, information, and rapid access to equipment and technology 
  • Workforce support for self-assessment and zero waiting times 
  • Integrated Care Teams using primary care for risk stratification 

To achieve this, we will support our social care workforce to work together and use community resources. We will continue to develop a workforce focused on innovation and connected to our communities. It's important that the social care workforce feels valued and can respond to the changing needs of our communities.
